Office Policies
Appointments Patients are seen by appointment only. Although we are not able to receive walk-ins, we make it a point to reserve enough space in our schedule each day to accommodate those with urgent problems. Please call ahead. We make every attempt to schedule you to be seen within 24 hours. For routine check-ups and visits, please call 4-6 weeks in advance so you may have a better selection of appointment times.
New patients If you are making an appointment for your first visit with us, please complete and print off a copy of the appropriate new patient forms to bring with you to that first visit. If, for some reason, you are not able to complete or print off the forms prior to your first appointment, please inform us so that we may send you the forms ahead of time. Please bring your insurance card(s) to all appointments.
Cancellations / Missed Appointments Please give 24 hours notice to cancel your appointment. Patients failing to keep their appointment may be charged a fee ($35 for missed routine appointment, $75 for missed comprehensive physical exam appointment).
Wellness Exams / Complete Physicals A wellness exam includes a thorough physical exam and screening for age-specific conditions, including cancers. Adults should fast for 12 hours prior to their exam so that the appropriate blood work can be obtained. Other appropriate tests may be recommended, such as a mammogram, EKG, colonoscopy, etc., based on that day's physical findings, your sex, age, personal medical history and family medical history. Infants and children will be given appropriate vaccinations. Most of the insurance companies with whom we participate cover annual wellness exams, but it is best for you to make sure of this prior to your appointment.
Because this is considered a preventative visit, insurance will not pay for other issues to be addressed. This includes chronic conditions like menopause, hypertension, allergies, etc. Likewise, acute problems like colds and bladder infections are also not covered during that visit. Please plan on addressing these types of concerns at an appointment on a different day if you want your insurance to cover them.
Insurance Information Please bring your insurance cards to all appointments. Should there be any changes to your coverage please inform us when you arrive for your appointment.
Billing & Payment We require payment at the time of service. We accept cash, check, VISA, Master Card and Discover. If your insurance requires co-payment we will request that at the time of registration. We electronically submit charges to insurance companies with whom we participate.
